Case Submission Consent & Media Release*
By submitting clinical case materials, including but not limited to photographs, radiographs, videos, clinical notes, treatment details, and written descriptions (“Submitted Materials”), I confirm and agree to the following:
1. Ownership and Authority

I confirm that I am the owner of, or have full rights and authority to use and share, the Submitted Materials. I further confirm that all materials have been obtained lawfully and in accordance with applicable professional, ethical, and legal standards.

2. Patient Privacy and Consent

I confirm that all Submitted Materials are fully anonymized and do not contain any information that could reasonably identify a patient, or that I have obtained all necessary patient consents required for educational and media use, where applicable. I accept full responsibility for compliance with patient privacy laws and regulations.

3. Permission to Use and Share

I hereby grant Augma Biomaterials a non-exclusive, royalty-free, worldwide, perpetual license to use, reproduce, display, publish, distribute, and discuss the Submitted Materials, in whole or in part, for educational, informational, and marketing purposes. This includes, but is not limited to:

– Podcast episodes (audio and video)
– Social media platforms
– Websites and online platforms
– Educational presentations and promotional materials

4. Editing and Commentary

I acknowledge and agree that the Submitted Materials may be reviewed, edited, cropped, narrated, commented on, or discussed by a specialist or host as part of the podcast or related content, without prior approval from me.

5. No Compensation

I understand and agree that I will not receive any financial compensation or remuneration for the use of the Submitted Materials.

6. Release of Liability

I release and hold harmless Augma Biomaterials, its owners, hosts, guests, employees, and partners from any claims, liabilities, or demands arising out of the use of the Submitted Materials as permitted under this consent.

7. Voluntary Submission

I confirm that my submission of the case and materials is voluntary and that I may withdraw from future submissions, but this consent will remain valid for materials already submitted and used. I acknowledge that my name will not be shared verbally or in writing.
