Implant Rehabilitation after Enucleation of a Large Periapical Granuloma

A healthy 67-year-old woman in good health presented to us with a request for restoration and prosthetic rehabilitation of the upper incisal region, with the desire for a fixed restoration.

She reported pain, suppuration, and bleeding in the area of ​​the right central incisor.

Radiography showed a mild radiolucency in the upper incisal region, more marked in the #11 (8) area.

On physical examination, the patient presented with a fixed prosthesis involving the four incisors from #12 (7) to 2.2 and the missing #21 (9). A probing depth of more than 15 mm surrounded #11 (8) distally and vestibularly.

Preoperative CBCT showed a large osteolytic volume from periapical granulomas expanding apically and vestibularly to the incisors.

The procedure proceeds with debridement and the application of biphasic calcium sulfate. After nine months, implant surgery is performed, followed by a subsequent six-month prosthesis with a provisional restoration under progressive loading, and a final fixed prosthesis after another six months.

The entire process was monitored with the ISQ to clinically analyse the behaviour of the regenerated bone.
